Located in the heart of Nebraska, Columbus Community Hospital (CCH) is a community-owned, not-for-profit hospital in Columbus, NE, that brings top-level health care close to home. The hospital is also supported by more than 230 volunteers. CCH is a 50-bed acute care facility that is certified for swing beds, with four skilled nursing beds and 14 ambulatory outpatient beds. CCH’s Healthpark Medical Office Building currently houses a visiting physicians’ clinic and the CCH laboratory, along with Columbus Otolaryngology Clinic, Columbus Orthopedic & Sports Medicine Clinic and Columbus General Surgery. CCH’s organization also includes a second campus, located south of Highway 30. South Campus includes medical outreach services such as Home Health and hospice, Healthy Families and Occupational Health Services.

Three options with different levels of deductibles, drug cards and co-pays. Midlands Choice PPO. Premiums - Wellness rate if employee completes wellness program criteria each year or regular rate if they do not complete wellness program criteria.

Staff contributes between 1% and 100% of income on a tax-sheltered basis. CCH match up to 4% max, after 12 mos, 1,000 hrs. worked, 21 yrs. old. Match schedule: 1-7 years of service - 4% match; 8-15 years of service - 5% match; 16+ years of service - 7% match. Your contribution will increase by 1% each year until it reaches 12%. Opt-out option available. Roth is an after-tax contribution.
Group Long-Term Disability Insurance

After 30-day wait, plan pays 66 2/3% of base salary, 3/12 Pre-Ex: Any claim incurred w/in first 12 months after eff. date would be denied if a condition exists w/in 3 months prior to the eff. date.
Voluntary Life Insurance

Term type insurance for employee, spouse, dependent children. Guaranteed amount if enrolled during first 30 days of eligibility.
Health Savings Accounts
Must participate in the CCH High Deductible Health Insurance Plan within 30 days of DOH (must be scheduled to work 24+ hrs per week)
Tax-free deductions for payment of un-reimbursed medical expenses. Annual contribution cannot exceed annual deductible. *$25 employer contribution each month. *Employee must contribute minimum of $25/per pay period to be eligible for employer contribution.
Aflac Insurance
Tax-free deductions for Critical Illness or Personal Accident Insurance. Benefits paid in addition to other insurance plans you may have.

Paid Time Off (PTO) .085 x # hrs/PP
Full-time/Parttime status only
Available immediately as accrued. Rate increases after completion of 7 and 15 years of service.
Shift Differential
Incentive for working between 6 p.m. and 6 a.m., 7 days/week.
Weekend Differential
Upon employment.
Incentive for hours worked between 6 p.m. Friday and 6 a.m. Monday.
On Call/Call Pay
Upon employment, not all staff eligible
Incentive for taking call.
Call Back
Incentive for being called back to work while on call.
Low Census
Upon employment, $3/hour, PRN staff not eligible
Compensation for scheduled work hours missed as the result of a decrease in work/case loads caused by lower than expected pt. census.
Emergent/Extra
Incentive for working an unscheduled/unexpected shift caused by an emergent situation.
Referral Bonus
Upon employment of referred applicant
$1,000 or $2,500 depending on the position recruited.
Holiday Pay
Incentive (time and ½) for working hours between midnight and 11:59 p.m. of the following holidays: Memorial Day, Independence Day, Labor Day and Thanksgiving Day. Time and ½ for working hours between 6 p.m. Eve before and 11:59 p.m. day of New Year’s Day & Christmas Day.

20% discount; payroll deduction for purchases in hospital cafeteria.
Tuition Reimbursement
After 3 months of employment. (must be scheduled to work 24+ hrs per week)
75%-100% reimbursement for college courses or certification up to $3,000 per calendar year.
Continuing Education
Upon employment (must be scheduled to work 24+ hrs per week)
Per situation, CCH annually pays 100% for required programs; up to $200 annually or $400 biannually.
Direct Deposit
Paychecks go directly to your bank account on payday. 26 pay periods per year; every other Friday.
Staff Health Services
Provides for initial and continuing tests and vaccinations.
Worker’s Compensation
Medical and income benefit for staff who suffers a job-related injury or illness.
Dry Cleaners
Centralized drop-off and pick-up within our hospital with Prestige Dry Cleaners (Second floor across from unit secretary desk).
College Savings Plan (NEST)
Payroll deduction available for those who want to invest in their child’s future.
YMCA Discount & Payroll Deduction
Upon employment or January 1st of every year. (Must be scheduled to work 24+ hrs per week to be eligible for additional cash stipend.)
Discounted membership through payroll deduction; $150 cash stipend every 6 months if 24+ hours/week (paid in July and January).
Exercise Program in Cardiac Rehab
Upon employment. Open enrollment December of each year
Open gym concept for employees/spouses. Gym is open 24/7. $5/month payroll deduction.
Child Care Center
Upon employment/availability
Licensed for 105 children between ages of 6 weeks to 13 years. Open 5 a.m.-7:30 p.m. Monday-Friday. Payroll deduction available for full-status CCH employees.
