Are you a Nurse Practitioner or Physician Assistant that has a passion to care for the Geriatric community? ChristianaCare is looking for a NP or PA to join our Primary Care at Home Team. The selected candidate will work in collaboration with our team of Physicians, APC's, RN's and Social Workers to care, evaluate, and manage patients in their homes as well as Assisted Living Facilities. This is a M-F position, with rotating on-call service after hours.
-Provides medical management of patients in the home, Assisted Living Facility, and skilled nursing facilities. This includes seeing patients in a professional manner, obtaining accurate history, performing a thorough physical exam, forming a diagnosis, developing, and executing appropriate treatment plans, and ensuring adequate follow-up care.
-Demonstrate skills and knowledge of common geriatric syndromes to provide appropriate care to patients. Common topics include but are not limited to dementia, delirium, urinary incontinence, constipation, ambulatory function, acute and chronic pain, wound care, polypharmacy issues, preventative care, health maintenance and palliative care. Analyze patient data and create an individualized care plan based on patient assessment and needs.
-Evaluate and monitor the effectiveness of care plan.
-Act as a liaison for patients, families, and health professionals. Actively communicate with the family on patients plan of care, progress, and prognosis.
-Appropriate referrals to social work, physical and occupational therapy for maintaining functional independence, Home Health Nursing for issues regarding on-going nursing management, palliative care, hospice care and other specialties for further management of acute/chronic disease processes.
-Consults with physician(s) on complicated medical cases
-Identifies end of life patients, has a basic knowledge and skill set for palliative care and appropriately refers patients to hospice care
-Fosters, maintains, and strengthens community relationships that benefit geriatric patients.
-Utilizing evidence-based medicine coupled with expert clinical knowledge to educate and promote well-being, preventative care, and chronic care management.
-Helps coordinate transitions of care to maximize quality of life and functional status for the patient and family
- Will take after-hours calls for the practice to reduce the need for inpatient and emergency department utilization and management of acute issues that cannot wait for until the next business day
-Provides nursing services including but not limited to immunizations, simple wound care, cerumen removal, and screening tests that are compatible with current nursing practice guidelines.
-Complete timely and billable documentation in the electronic health record (EHR), which includes but not limited to history and physical, referrals, patient encounters, phone calls, family discussions, changes in plan of care, orders, and message center.
-Submit billing / coding in a timely fashion (reference policy, if applicable)
-Contribute to program development and utilization of best practice guidelines, quality, and performance/process improvement.
-Provide excellent customer service and assist in efforts to improve patient satisfaction.
-Strive for the highest quality in all aspects of work performance and assume responsibility for improving care/services.
-Maintains privileges, mandatory education, and regulatory requirements as needed for area of specialization
