The RN will work in the Mental Health Division as a member of MH-PC Integration Team. This team is collocated in the Primary Care area. The RN works with the members of the MH-PC team to coordinate care of veterans referred from Primary Care for depression, PTSD, or other mental health diagnoses. The PCMHI RN will function as a Depression Care Nurse, providing care management services to those with a diagnosis of depression as well as monitor those with other Mental Health diagnoses. The PCMHI RN possesses the knowledge and skills necessary to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. This broad based clinical knowledge enables the PCMHI to effectively implement health promotion and prevention interventions, manage acu
