DescriptionSummary:Reporting to the Clinical Director of the Neonatal Intensive Care Unit, this person is responsible for coordinating the discharge of patients in the NICU. In partnership with the Medical Director, NICU Leadership, medical provider, nursing staff, case managers, social workers, physical/occupational/speech therapists, child life specialists, pharmacists, dietary, chaplain services and High Risk Infant Clinic coordinator, works to provide a smooth transition from inpatient to outpatient care for NICU patients and their families.
