Summary

The Social Worker provides an integrated continuum of care for patients from admission to post-discharge. The Social Worker screens for discharge planning needs and initiates a plan for community resources and/or placement in appropriate post-discharge facility. The Social Worker promotes safe discharge, appropriate length of stay, readmission prevention and patient satisfaction.

Education
MSW Required, LCSW preferred

Experience
2 years acute care experience preferred

License
LCSW for CF or Dialysis

Certification
BLS (AMERICAN HEART ASSN)

Candidates in unit have priority based on unit seniority
