In this position you will be responsible for providing high-touch, patient-centric care to drive an exceptional experience for patients enrolled in the ChristianaCare Specialty Pharmacy Program. You will be a primary pharmacy resource for providers within the pulmonology practices, provide longitudinal medication education and monitoring to patients enrolled in the ChristianaCare Specialty Pharmacy Program, and provide medication-related care for the purpose of achieving defined outcomes as specified within health system protocols and policies.
