This position is located in the Primary Care unit at the Nashville VA in Nashville, TN. May need to travel to Outpatient Clinics. RN Pay Scale NG 57 A general description is: The PACT Coordinator is a Registered Nurse who serves as facility champion and coordinates local PACT implementation, training, communication, outreach and sustainability of the PACT model. The Coordinator plans, organizes, directs and evaluates the PACT program and reports related actions/activities to facility, VISN and national VA leadership. The incumbent serves as the PACT Program Planner and Site Coordinator. These functions involve active promotion of the PACT model among clinical and administrative staff within the Ambulatory Care Setting, educating an array of stakeholders; identify/addressing communication
