Providence Medical Group (PMG) Value-Based nurse practitioners are an integral member of Providence's care teams, serving as a knowledgeable expert in preventive care, risk assessment, and population health leadership. In this role you will see patients for their Annual Wellness Visits which include review of medical and family history, limited physical exam checking vital signs, screenings to detect depression and cognitive impairment, review of functional ability, risk for falling as well as review for preventive care and provide education and resources/recommendations for wellness services and health lifestyle changes. You'll collaborate and serve as a resource to the patient's primary care team as they monitor health conditions and update plans of care. In this position, you will also have time dedicated to providing thorough and detailed chart review, identifying conditions that require clinical assessment, or any gaps in care.
