Duke Hospital (Population Health) Durham, North Carolina

The clinical pharmacist will provide innovative pharmacy services to patients served by the Population Health Management Office (PHMO), including Duke Connected Care's (DCC) clinically integrated network. Specifically, the clinical pharmacists will assist PHMO interdisciplinary teams in improving quality metrics, preventing inpatient admissions or readmissions, and decreasing unnecessary utilization costs. The clinical pharmacist shall serve as a resource for healthcare providers and staff. A clinical practice may be maintained at an outpatient clinic. The position also requires precepting UNC pharmacy students and Duke Pharmacy residents.
Clinical Pharmacist Responsibilities
· Participates in clinic and care management activities to provide clinical pharmacy services and oversight for safe use of medication. This may include prescribing and monitoring of medications within the scope of practice designed collaboratively with physician colleagues. This may include, but is not limited to:
o Completes/Reviews medication reconciliation.
o Provides medication counseling.
o Prescribes and adjusts medications as permitted through collaborative practice agreement.
o Screens for drug interactions, therapeutic duplications, omissions, etc.
o Monitors patients for therapeutic and/or adverse response to medication
o Coordinates patient care with other outpatient or inpatient teams.
o Ensures continuity of care and access with all medications.
o Documents the care provided in patients' records.
o Participates in multidisciplinary reviews of patients' progress, when applicable.
o Serves as resource to PHMO and DCC providers, care managers, and other healthcare professionals to provide medication reviews and to assist in accurate completion of medication reconciliation activities.
o Assists in identifying and developing interventions for patients that are not meeting target quality measures.
o Serves as resource to DCC network providers and care managers to assist in addressing medication access and medication adherence challenges.
o Refers patients to patient assistance programs, or other resources to navigate medication access issues, as needed.
o Proactively assesses and encourages appropriate medication adherence.
o Serves as a resource to providers and staff for timely general drug information and payer pharmacy policy.
o Provides educational in-services on medication-related topics to patients, providers, staff, care managers, and other healthcare professionals.
o May provide discharge counseling and education for select high-risk patients, either face-to-face or telephonically.
o Provides both direct patient care and virtual services to patients as determined based on oversight from applicable clinic, PHMO, and/or pharmacy leadership.
o Interprets data related to medication safety and effectiveness
and provides support to improve safety and reduce risk to patients. Reports events and/or concerns via the Duke Health Safety Reporting System (SRS).
o Provides oversight of cost-effective use of medications and treatments. This includes risk reduction and correct utilization of resources to optimize care.
o Participates in developing guidelines, algorithms, and care pathways to improve the care and safety of patients.
o Participates in efforts to maintain TJC accreditation standards within the realm of medication use at clinical practice sites
o Represents specific concerns for Information Technology applications.
o Facilitates prescription fulfillment through coordination with retail pharmacies.
o Develops and participates in pharmacy or medication-related related research projects and quality improvement initiatives.
o Educates medical, nursing, and pharmacy staff about the management, prescribing and monitoring of drug therapies.
o Provides educational lectures and/or participates in research (Duke, State/Local, National).
Participates in Duke Health committees and initiatives related to population health, transitions of care, and/or pharmacy practice.
Collaborates with peers to achieve department and organizational goals.
Coordinates with others to maintain up-to-date knowledge of pharmacy benefit design and access in support of pharmacy practice.
Maintains familiarity with pharmacy programs across Duke Health and functions as an integral member of the pharmacy team.

PGY2 Ambulatory Care Residency and BCPS or BCACP preferred, 1-3 years of clinical practice experience preferred, and a demonstrated ability to achieve positive patient outcomes and possess strong interpersonal skills.
Doctor of Pharmacy Degree OR BS Pharmacy Degree
With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required. With Doctor of Pharmacy degree, no experience is required.
Licensed Pharmacist or eligible to practice pharmacyin North Carolina
