This clinical pharmacist will maintain a clinical practice and actively contribute to the continuity of care for a designated patient population as they receive outpatient infusion treatments.
Monday-Friday, 8am-5pm no holidays or weekends
Clinical Pharmacist Responsibilities:
· Management of all aspects of day to day drug distribution and delivery services including staff scheduling, inventory handling, safety monitoring and technology oversight to ensure timely and accurate delivery of medications to our patients.
· Responsible for the supervision of team members which may consist of pharmacy technicians, and ensures appropriate training of staff in their job functions.
· Responsible to obtain and maintain licensure status of the pharmacy with the North Carolina Board of Pharmacy (NCBOP)
· Responsible for routine inspections from NCBOP, and medication management aspects of accreditation visits from Joint Commission and NCDHHS.
· Responsible for maintence and documentation of applicable USP <797> and USP <800> standards.
· Review and monitor patient's medication therapy and orders written by appropriate caregivers for safety and efficacy considering specific patient conditions and health status; advise patient care team of issues with, or possible enhancements to, patient's medication therapy and document such interventions.
· Prepare, label and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records.
· Maintain appropriate records of dispensing and oversee and maintain the systems that charge or credit for medications and medication treatments.
· Provide drug information and educate caregivers, patients and students regarding medications and their proper use and monitoring.
· Ensure that appropriate medications are available for patients and properly procured, prepared, stored and maintained.
· Supervise the process and technical personnel who assist in preparation, maintenance and dispensing of drugs and supplies.
· Participates in clinic and provide clinical pharmacy services and oversight for safe use of medication. This includes prescribing, dispensing, administering, and monitoring of medications. This may include, but is not limited to:
· Provides medication counseling/education o Interprets pharmacokinetic/therapeutic drug monitoring data
· Screens for drug interactions, therapeutic duplications, omissions, etc.
· Monitors patients for therapeutic and/or toxic response to medication
· Ensures continuity of care and access with all medications
· Reports events and/or concerns via the safety reporting system.
· Provides oversight of cost-effective use of medications and treatments. This includes risk reduction and correct utilization of resources to optimize care.
· Provides leadership to maintain TJC accreditation standards within the realm of medication use within the Program.
· Collaborates with Infusion Pharmacy staff to provide continuity of care on medication infusion related issues as necessary
· Provides staff education for critical elements of medication use in patient care
· Educates medical, nursing, and pharmacy staff about the management, prescribing, and monitoring of therapies.
· Participates in the education of pharmacy students and pharmacy residents.
· Understands and actively participates in the achievement of division and departmental financial goals
· Maintains familiarity with all departmental programs and works as integral part of the pharmacy team.

· Takes responsibility for personal and professional growth
Doctor of Pharmacy Degree OR BS Pharmacy Degree
Two years of pharmacy practice experience; Managerial/supervisor experience preferred
Licensed Pharmacist or eligible to practice pharmacy in North Carolina
