GENERAL SUMMARY: The Brain Services Navigator facilitates an evidence-based, multidisciplinary approach to neurosurgical care throughout the care continuum at the campus. Provides direct clinical service to patients and families including care management and patient education to understand diagnosis, treatment and follow-up regimen. Provides education and mentoring to advance staff knowledge at the campus. Gathers, abstracts and reviews data for process and quality improvement. Help and oversee the navigation of patients/families through the medical system and ensures timely and appropriate evaluation, treatment, educational services; and access to appropriate resources, as needed. May facilitate/ coordinate patient support and/or educational groups and continually strive for innovative and creative strategies to impact and improve patient quality of life. Assists in program development and expansion. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
PRINCIPAL DUTIES AND JOB RESPONSIBILITES: • Formulates an individualized care plan based on therapeutic principles as appropriate for age/psychological development; evaluates the effectiveness of nursing/medical interventions and makes modifications as needed in accomplishing desires patient outcomes; assists in the assessment of the patient with TeleMedicine as needed. • Initiates patient coordination at the patient’s entry point into the system. Prepares patient case for pre-procedural review with multi-disciplinary team. Ensures patient case follows clinical path prescribed/therapeutic interventions via case review process. • Navigates appropriate neurosurgical inpatients from the admission diagnostic phase through the treatment and discharge phase, ensuring appropriate/timely medical/supportive services; good organizational skills in planning/implementing daily activities, and facilities early discharge of patients. • Communicates effectively with physicians, healthcare team, patients and families to ensure coordinated and cohesive execution of evidence-based quality patient care; establishes/maintains a therapeutic relationship with patients/families. • Serves as patient resource and advocate, promoting patient access to available services, tools and support. Develops, coordinates and documents patient education. • Performs follow-up phone calls after discharge and contributes to post-acute care planning; assesses patients’ knowledge and understanding of discharge plan and instructions; provides/reinforces education as needed. • Gathers, abstracts and reviews data for process and quality improvement; analyzes practices that contribute to variance, draws conclusions from data and assists in developing action plans utilizing best practices and research findings. • Researches and responds to all Neuro-Oncology Tumor Board related information requests. Records all conversation at weekly Tumor Board conference, including patient history, diagnosis, research outcomes and treatment recommendations. Follows up with outcomes as necessary. • Ensures that patient’s physical, mental, and spiritual needs are met by coordination with appropriate care team members. Maintains a system for coordinating and tracking the continuum of care for patients within the system. • Assumes responsibility for professional growth and demeanor; attends all mandatory nursing/hospital in-services, maintains and updates knowledge/skills of current trends, practice changes, and research findings related to the neurosurgery patient population. Maintains knowledge by attending professional workshops and networking with other professionals. • Assists with program development; creating a system wide program that expands referral base to include all of AdventHealth; actively involved with marketing strategies and meetings, maintaining relationships with referring physicians. • Assists in the development of annual marketing plans in collaboration with the physicians, consumer marketing, and leadership • Establishes relationships to maximize program effectiveness and facilitates problem resolution across departmental, campus and physician/provider service lines. • Establishes and maintains knowledge and ability to assist in the Gamma Knife operational department, providing care to the Gamma Knife patient.
KNOWLEDGE AND SKILLS REQUIRED: • Advanced knowledge of adult neuroscience, neuro-interventional, brain related treatment options • Skilled in providing age appropriate education and patient care by interacting positively with patients, families, staff, physicians and the community • Strong communication and collaboration skills • Maintains the confidentiality of patients, physicians, and department information with no infractions • Aptitude for critical thinking, decision making, and managing competing priorities • Ability to interact appropriately with all other departments regarding care management and illness adjustment issues • Skilled in self-direction and organization. Effective decision-making skills with minimal supervision • BLS, ACLS KNOWLEDGE AND SKILLS PREFERRED: • Data analysis • Program development • Case management • Process/performance improvement EDUCATION AND EXPERIENCE REQUIRED: • Bachelor’s degree in Nursing or Health Administration • Three to five years’ experience in neuroscience nursing or related healthcare field EDUCATION AND EXPERIENCE PREFERRED: • N/A LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED: • Current valid State of Florida license as a registered professional nurse LICENSURE, CERTIFICATION, OR REGISTRATION PREFERRED: • Certification in Neuroscience Nursing (CNRN), Critical Care Nursing (CCRN)
