Job Summary:
The Manager for Cardiovascular (CV) System of Care reports to the Director of Ambulatory CV Services. The Manager is responsible for directing the provision of quality Cardiovascular virtual and ambulatory services delivered by an interdisciplinary team in a comprehensive, collaborative manner with an emphasis on health promotion, avoidance of readmission, chronic disease management and education. As well, that no patient is ever truly discharged. The Manager develops and is accountable for the activities of the various program and its staff; represents and promotes the CVI by participating in various community activities; and is a key leader in the overall management of the virtual and ambulatory services of patients.
The Manager works in collaboration with the Cardiovascular Institute (CVI) Team member's primary care manager (CM) to ensure safe and appropriate transition between levels of care, including working as a part of the interdisciplinary care team in the development and execution of a person-centered transition plan. Coordinates with support staff, (e.g., Housing Outreach Specialist, Education Specialist, Virtual Care, DME Specialist, and Community Advocates) and services required for a safe transition. Along with the primary CM, collaborates with family/caregivers to identify and ensure member and/or caregiver needs are met and services are in place to support a successfully achieving transition goals. Assists with identifying resources with specialized capabilities when members with complex needs transition to the community, residential facility or group home. Identifies members residing in nursing facilities or other institutions who are candidates for transitioning back to the community. Assists in reviewing and analyzing data related to rebalancing and developing action plans to meet patient care goals. Serves as the liaison for pre- and post-acute care for cardiovascular conditions including all CVI programs.
Provides administrative support in the areas of human resources, finances and general accountability to the Director.
The Manger will have a large geographical area of responsibility that includes both physical and virtual that will evolve from Central Florida upwards to a large regional scope.
