Fairview Health Services is looking Transplant Coordinator to work on the University of Minnesota Medical Center Campus in Minneapolis, Mn. This is a full time benefit eligible position.
The Transplant Coordinator coordinates all activities related to receipt of organ offers for kidney, pancreas and liver donors. Works in collaboration with the transplant surgeon to evaluate suitability of organ donor and potential recipient. Collaborates with inpatient teams to coordinate transplant surgery and timing of donor organ procurement.
Collaborate with MD, interdisciplinary team members, community members, patients, families, manage, evaluate care systems and resources.
Serve as liaison with transplant physicians to implement medical management of condition changes, diagnostic test results, lab results, based on knowledge of patient and patient condition.
Acts as liaison with community MD and pharmacists/home care/IV to ensure continuum of care is provided.
BA/BS in Nursing
3-5 years nursing experience in acute care, ICU, CCU, Transplant, Acute Med/Surg
Experience:3 years of Transplant Coordinator experience.
