Reporting to the Nursing Director of the Neonatal Intensive Care Unit, this person is responsible for the day to day operations of the Developmental Clinic. Ensures coordination of the developmental clinic activities in partnership with the Pediatric Developmental Specialist, Medical Director, Nursing Director, existing Neonatal ICU staff, case managers, social workers, physical, occupational, speech and child life specialists, dietary and chaplain services. Enhances the continuum of care for the clinic's patient population and provides a smooth transition from inpatient to outpatient care for the patients and their families. Provides case management of the chronically ill patient caseload to include but not limited to resource recommendation, referrals to various community agencies, appointment scheduling, coordination with ancillary departments and maintenance of documentation/paperwork required to support the plan of care. Performs the duties of the Neonatal Intensive Care Unit discharge rounds nurse one day a week.
Bachelor's of Science in Nursing from an accredited School of Nursing preferred.
Will consider experienced nurse with Associate of Applied Science or Diploma degree.
Minimum five (5) years of Level III neonatal nursing experience required.
Case Management experience and/or coordination of services experience required.
